ISO-11179 Definition
A set of HEDIS measures assessing the quality and completeness of care transitions between settings including notification of inpatient admission to the primary care provider, receipt of discharge information by the member and receiving provider, patient engagement after discharge, and medication reconciliation post-discharge. Transitions of care measures collectively assess health plan effectiveness in coordinating care across the hospital-to-home transition, with poor performance associated with higher readmission rates and post-discharge adverse events.
