The association between an individual healthcare provider and a hospital or health system at which the provider holds clinical privileges to admit and treat patients. Hospital affiliations are a key credentialing data element verified during provider network onboarding, as they confirm the provider has met the hospital medical staff standards for clinical competency. Providers may hold affiliations at multiple hospitals.

Health plan provider directories display hospital affiliations to help members identify which hospitals their physicians are associated with for inpatient care planning. Healthcare data teams maintain provider hospital affiliation records linking individual provider NPIs to facility NPIs with affiliation effective dates, privilege types, and active status, using affiliation data in network adequacy analysis to map inpatient care access and in member communications to help members navigate hospital-based care.
