A healthcare institution or physical location where clinical services are delivered to patients, including hospitals, ambulatory surgery centers, skilled nursing facilities, rehabilitation centers, urgent care centers, dialysis facilities, and behavioral health facilities. Facilities are distinct from individual practitioner providers in that they bill under Type 2 organizational NPIs and are subject to facility-specific credentialing, licensing, accreditation, and reimbursement rules. CMS maintains separate enrollment processes and fee schedules for facility providers compared to professional providers.

Healthcare data teams maintain provider facility records with facility NPI, facility type code, accreditation status, bed count for inpatient facilities, and network participation status to support claims adjudication, network adequacy analysis, and member-facing provider directory accuracy for facility-based care settings.
