A HEDIS measure and CMS Star Rating indicator tracking the rate of unplanned acute inpatient readmissions within 30 days of discharge across all members of a health plan, regardless of principal diagnosis, used to assess care coordination effectiveness at transitions from inpatient to post-acute care settings.
