An exclusion criterion that health plans may choose to apply or not apply to a quality measure depending on their data availability and reporting strategy, allowing removal of members with certain conditions such as frailty, advanced illness, or dementia that may make the measure clinically inappropriate. Unlike required exclusions, optional exclusions give health plans discretion, but their consistent application must be documented and maintained across measurement years for audit validity.
