ISO-11179 Definition

The five-digit postal ZIP code of a health plan member primary residential address as recorded in the payer enrollment system. Member ZIP code is used in geographic risk adjustment, network adequacy analysis, care gap identification for local provider outreach, and social determinants of health analytics by mapping to census tract and neighborhood-level data. Under HIPAA Safe Harbor de-identification standards, ZIP codes with populations under 20,000 must be truncated to the first three digits.

Healthcare data teams store mbr_zip_cd as VARCHAR(10) to accommodate ZIP+4 extended format and avoid INTEGER data types which drop leading zeros for northeastern ZIP codes beginning with zero.
