Definition

ISO-11179 Definition

The ratio of incurred medical claims to earned premium revenue for a defined period, equivalent to the claims component of the medical loss ratio before including quality improvement expenses. MER is the primary measure of underwriting profitability for health plans, with ratios below target indicating favorable claims experience and ratios above target indicating adverse experience requiring premium adjustments, utilization management intervention, or provider contract renegotiation to restore profitability.
