Definition
The count of distinct disease conditions or diagnoses recorded for a patient within a defined period. Used in risk stratification, HCC scoring, and population health analytics.
Risk Adjustment Context
Disease total is a key input for risk adjustment models including CMS-HCC V28 and CDPS. Higher disease totals correlate with increased RAF scores and higher capitation payments in Medicare Advantage.
