The systematic identification, monitoring, and coordination of care for members with defined chronic conditions including diabetes, hypertension, heart failure, COPD, asthma, and depression, using evidence-based clinical protocols to improve disease control, reduce complications, and prevent acute exacerbations requiring hospitalizations. Chronic condition management programs use quality measure data to identify members with poorly controlled conditions and target care management interventions to those with the greatest clinical need and improvement potential.
